Hi

I would like to know whether it is more likely than not that the HIV duo test would pick up a group n infection even having taken the test a year later after exposure so that p24 antigens were no longer present. The only reason i mention this is due to the fact i visited a brothel in south spain and the woman was black african. Both my hiv isnti and hiv duo test were negative taken over 6 months after the exposure. I am concerned because my girlfriend developed an allergic rash with no other symptoms after i had been tested negative after unprotected sex.

Thanks for your time

Morning Doctors.
I wonder if you could help me please. I am female. My exposure - I performed unprotected oral on a male friend of mine a few times.

I know this is low risk but have had all the tests done and happy with the negative results as I know there are definitive window periods for these, however for Hep B it seems a bit vague.

I think I've done far too much reading on the internet. The CDC states that 1-9 weeks for the antigen test can be considered reliable whilst I have been reading on some forums that results can only be considered conclusive after 6 months!!!!

This is what I have had done:

7 weeks - Hep B surface antigen - negative (private clinic)
10 & 12 weeks - HBcAB - negative (gum clinic). The gum clinic only do the HBcAB test. The nurse said that at 3 months this would be conclusive, however when I called the results line, the advisor said 6 months!!
10 & 12 weeks - Hep C and syphilis - negative

I really need to move on from this and have been able to let the other testings go as I know they can be considered reliable but for some reason, the Hep B testing window seems vague.

However yesterday I found this reply from Dr Jose:

"Hep BsAg could be tested after 4 weeks of the encounter and a negative result would be conclusive. At 12 weeks, a negative anti-Hep Bc Ab can also be considered conclusive"

I know these tests are known as different names - I assume the Hep BsAG is the surface antigen and that the anti-Hep Bc AB is the HBcAB? If so then surely I must be ok as I've had both done?

Sorry if this is a bit drawn out but just wanted to explain fully my quandry and hope you can help me put this to bed. Do I need anymore testing? Thank you so much and happy to have made a donation to worthwhile causes.

Hi doctors,

I am an uncircumcised gay male (27y/o) from Hong Kong. Around 3 days ago (Sept 2 midnight) in a gay sauna I had mutual masturbation with a stranger guy, inside a dark room. By mutual masturbation, I meant one touching one's penis and then touching another's penis, using the same hand. When I touched his base and shaft of his penis, it was somehow slick and wet. I didn't know what it could possibly be - I am thinking towards the worst that it could be his semen (coz I saw him fooling with other guys right before me). During this encounter, he touched his penis and then touched mine instantly using the same hand. I did the same to him as well. I ejaculated in the end. Then I had a shower and left the sauna.

I have the following questions:
(1) Would I be at risk of HIV infection, assuming that somehow his pre-ejaculate or even worse his semen got onto my penis during mutual masturbation (as lubricants by accident)?
(2) Would I be at risk of genital herpes/genital warts infection by mutual masturbation as described in (1)?
(3) Right now it's almost day 4 from the encounter. I still have yet to find any classical symptoms of genital herpes (painful feeling, redness, blisters, sores). But I always have some itch around the area. How painful or itchy would I feel preceding any classical symptoms? When would the symptoms emerge for the first episode of genital herpes attack? Is it around 14 days?
(4) Should I get tested for genital herpes (assuming there's no symptoms, God bless)? Is there any test for genital warts as well?

Thanks

Dear Dr.,
it is now exactly 12 weeks after my exposure with a very brief (3min and now cum involved) unprotect anal sex. I need your advice since i am a bit puzzeld and lost :( here a summary:
day 29 after sex: light fever of 37.3, sore thoath and swolen glands
day 47 after sex: neg. hiv rapid test
day 58 after sex: neg. Determine HIV rapid test
day 68 after sex: also neg. rapid test

I still have slight temperature with 37.0 not all days but continously...

Now at date 84 i wanted to test again with Determine HIV rapid test. However, it was strange the blood sample didnt dilute completely through the test as it did always before (and i did this twice and always the same reaction). I waited the time but only half diluted. Can it be that the blood reacted to HIV reagens and therefore was blocked and couldnt dilute? Have you had seen that before? The blood what diluted (i would say less than 50%) did lead to a strong control line and no test line. Is the test still valid?!

Therefore my questions:
- is there something wrong with the test or is it due to now finally HIV antibodies being picked up, have you experienced that ever before?!
- am i irrational and should stop worrying since my test at day 58 & 68 are conclusive already?
- what is your experience with rapid tests after that period of time becoming positive?

Thank you soo much for any help and advice!!

Dear Doctors,

I am extremely worried about HIV. I haven't been in any unsafe sex since the last 10 years ago... (I mean I haven't do any sex with strangers only with my wife).. I did HIV tests twice last month. the first one is in the clinic in the hospital I think it is called GUM (the main hospital).. and the second test (INST test) the one that takes I think 60 seconds for the result I did it in your clinic with Dr Jose on the 5th of August. I think he may remember me because I told them that I was doing my dissertation and I read an article about AIDS and from that time I have been worried.. both tests are negative. although Dr Jose assures me that I am fine and this worry results from the pressure that I was under while I was writing my dissertation. After both tests, I was quite worried about HIV I tell myself..(I might get infectet by the injections that the GUM uses for the test or that one that is used with the INST test the one that is used on my finger... I know that this is impossible but I do not know why these thoughts are clicking into my mind...

since the last two weeks I have been suffering from:

1- nasal congestion.. one of the nostril is blocked and sometimes it opens and the other one becomes blocked but when I get out of the flat the blocking of the nose relieves I went to the GP and they said to me that there is inflammation in the nose (red colour).. I sneeze most of the times particularly in the morning when I get up I always get up in the early morning.. around 6 o'clock or sometimes 4 o'clock.. the nasal congestion starts since the last two weeks or three and I am suffering from it till now. but I use nasal spray (Sudafed) )which the GP said to me this will not cure the inflammation but it helps to get rid of the congustion for 6 hours and now since today I am using Flixonase which the GP sai that may take time to cure the inflammation.

2- sometimes I feel pain on the shoulder.. I have suffered from that many times before the test and which I think from the cold particularly when i get in the morning because sometimes I do not cover myself with blanket although I did before I sleep but when I am sleeping I move it unconsciously

2- I have not suffered from sore throat but there is a pain in the soft palat but this pain is not when I eat or drink..no.. I eat and drink without any pain in the throat I mean the pain is not as same as the pain of the sore throat..I went to the GP three times and they ckecked that my throat does not have any sign that it is sore or fatigued I do not have it. I asked my friend and he said to me this is due the nasal passage from inside the mouth not from the throat because he had suffered from nose allergy before and this is from experience...
I am not sure whether I am right or wrong..so please correct me,
sore throat is a pain that a person feels when I eat something or drink and they could not drink or eat easily they suffer from a pain.. I am not suffering from this sort..

2- yesterday, I took a shower in the eveinging around 5 o'clock which started with warm water till I felt that my muscles slept and then I increased slightly the cold whiich is not that cold but I moved from hot water to somewhat cold (slight cold I mean it is more than warm water in a digree or so) and then I stayed in the TV for three hours while I was well dressed I think when I got up I suffered from Diarrhea.

3- yesterday before I took the shower while I was out in the town center I felt a slight pain in my Abdomen.

4- as for the tempretaute, I always check my tempertature by using the thermoscan (Braun) throught the ears which is similar to the one used in the GP.... my temerapture ranges from 69.9 C to 75 C... and I don't feel that I have a high temertature due to two reasons because i always check it and I feel I can walk and do my housework and read and my everyday activities seem to be okay..in some hours I feel I am shivering when i took the temperatur I found it is 69.9C

I do not suffer from sweating

5- I am suffering from itching on two areas onely (i) the back of elbow and (ii) the back of knee. I read about on the internet it says this is callled eczema and when I scratch the area becomes red (rash) I do not have rash covering my body>>> my body looks the same as the one before the test..

finally,, due to worries, I always read about HIV most of my times and read about symptoms and experiences
I am not alcoholic and I never drink alcohol and I am not a drug users, even I have never ever seen them
My questions:
Am I in a risk of HIV due to these symptoms? and
why am I suffering from these symptoms..Is it really due the cold and I am suffering from the cold not HIV?

Can I be infected in the GUM in the UK while they do a test there because there are many people going to this place to do the test which it might be one may be infected and the nurse uses the same injection with me or something lik this..I know this is impossible but I do not know why I am thinking of this.. and these symptoms I am sufferring increase the thoughts.

please help me

thank you

Hi Dr Jose Dr Sean

This is a bit long winded but hear gos
Ive Spoken with Dr Jose and Dr Hook on medhelp my user name there is stupids
I was worrying about HIV but both docters have told me i dont need to worry about that, trouble is im still worrying and if its not HIV then what could this be?
Im going to list as much as i can to try and paint the full picture
Back in may i went for a full health check up as i had been experenceing slightly lumpy sperm and loose stools for about to years, they took blood test which came back with slightley raised white blood cells , whould this make me moore prone to picking up other infections as it seem i am alredy fighting one.
now on with the present symtoms and timeing of what im now experencing
now for the un risky regarding HIV exposer
29th of July (thursday) I visited a sexworker from the adultwork website she was fairly old about 50 and was pretty skinny i perfomed cunnilgus on her for about 15 mins and she sucked me off and deep throut for five mins i came in her mouth

on 30th friday morning i felt a slight discomfert under my tounge and wonderd if i had split the little membrain underneith it as this was where the discomfert was comming from tounge beter the next day

31st july/ 1st august sat sun ,i had sex with my wife for while i gave her oral as well as being sucked i did not cum inside my wife insted pulled out and wanked the last bit and came over her chest and face

tuesday the 3rd of august / slight pain in throat and coughed up some clear phlegm ,couple of realy hot flushes that dont last long but not feeling quite right, started worrying

thurday the 5th start getting alot of rumberling in tummy followed by loose stools , throat pretty sore and a cough with the plhegm clear but clowdy looking feeling tired

friday the 6th start worring and take myself down to the gum clinic, he said my throat looks sore and red and said it looks like and infection but not nessarsely a sexualy transmitted one, took a swab for gonaria and peed in a pot , result from this negative

Saturday 7th im off my food and just not feeling myself cough is now pretty persisdent and dry

Sunday 8th spend most of the day laying on the couch , no appeitite , dry cough gearal feeling of being unwell

monday the 9th force myself to go to work , by lunchtime start feeling alot beter but still have cough
tue/ wens / not to bad still not quite right still got cough

thurday12th not to bad still not hungry but eating, sudern need to go to the toilet very loose stool not watery direre but very runny like a cow pat also start getting conjuntvitis in only one eye could be due to getting something in eye at work

friday13th out for a fryed breakfast but not up to finishing it all unlike me, eyes still bad still got cough thick yellow phelhgm in the morning and clear frothy phelghm later in the day this gos on for a few days

wednesday 18th still got conjuntivis but now in both eyes, phone for results for swab at gum clinic negative and tell them about my eyes , they ask me to come back in on friday

friday20th go to gum clinic he swabs eye and throat as he said clamidia was hard to detect in throat and it was not tested for before only in urine which was clear, told him im worrying about HIV and he said not a concern in your case oral is not a risk of infection. been told that before by DR Jose and by dr Hook he precribed me with doxy 100mg twice a day for a week he said just a precoution.

saturday 21st body and face start to get rather itchy nose chin behind ears and iner ears ,this is still with me today maybe slightly beter but still there

Sunday the 22nd my wife falls ill bad sickness and direre stays in bed for a whole day dose not eat a thing, this lasted 24hour and then she starts to feel beter, my eyes have now sorted them selvs out and the cough has finally gone but im still feeling under the weather

Tuesday the 24th going away for a few days with the wife and baby she tell me her neck herts today, myn was also herting but i did not want to worry her as ive already worried myself sick about all that been going on

today 3rd of september
got back of holiday last night, neck still feels abit stiff and has done for the last ten days throat still feels abit quere not to sore but not right , i think but dont know 100 pecent that my neck glans are up neck herts mainly when turning to the sides , it is not as bad as it was but still unconfertable to say the leesed itchy face and body mainly face is getting me wondering what the hells going on most days i was away i felt tired and could not wait to lay down on the soffa after a day out,

ok so there we have it main symptoms for me are
lack of appetite
anoying throat coughs gone at the mo
achy neck
conjuntivits for a week
feeling tired all the time
swolen neck glans? maybe but i think so
itchy face and body ,nose realy itchy and behind ears
second to last day of holiday i found a very small rash in a couple of places arm and chest only small areas but enough to make me worry about HIV again ,its gone today
Dr Jse i no youve told me twice that i need to stop worrying but all the time im getting symtoms i worry, i need to stop worrying about HIV as you and dr hook and gum clinic say its not a risk, i even got throan off aidsmed forum for asking about symtoms they said no risk of HIV and find out what is realy wrong if any thing, you also told me to mabe get tested just to stop me worrying and for no other reason, I beleve you
What else could be corsing these syptoms as they seem preety real
I know anxiety and stress from worrying could maybe play a part but what else could corse so many different symtoms?
and this itching when i googled it it says sem dem is common with poeple with aids what if it is seb dem and i have gone from ars in to aids already, i dout that i just thought it so thought i would ask. as the itching is very real
if this was ars its gone on for to long now , right?
sorry for such a long post but wanted to let you know everything
sorry again one more thing when my wife gave birth she was told she had group b strep if i gave her oral could i catch strep throat and would this be acounting for the symptoms?
Once again sorry for this long post and thankyou both so much for running a site like this, i will come to freedom health for a test at some point but i have a few things going on at home in the next month and i dont wont to get tested just yet as it might spoil the events back hear if i was to be the first reported case of HIV via oral exposer
Thanks Thanks Thanks

Hi

14 weeks ago I gave oral to a guy who later told me he was positive, there was no sperm or blood in my mouth & and my oral health is ok

I was paranoid so at 28 days i had a insti test = negative.
A little while after I came dowm with a bug/virus, i was weak felt sick, aching
so at 35 days i took a duo test p24 test = negative
i continued to feel weak /sick, went to see my doc but he found nothing
at 80 & 90 days i took further insti tests both negative

My question is as I still fell weak/nauses 10 weeks after I firsted started to feel ill, do you feel further testing is necessary?

Or should I leave this to my GP , who thinks I do not have a physical illness

Can we consider the test I have as conclusive, and the bug i caught as just coincidence?

Dear Dr Sean/Jose,

I have been advised by a patient of yours to use this forum for some answers.

I am a 32 years old married male with two kids.

A week ago, one of my closest friends told us that he was diagnosed with hiv. He probably have it for more than 6 months since the source of infection was identified to be his ex-girlfirend (they broke up 6 months ago).

I am really scared since we are very close friends and we spend a lot of time together. He told us that he will continue his life as before (play football with us, go out, watch football matches etc.). I am not sure if I could still spend so much time with him.

I do not want to discuss my worries with him. I try to behave normally in front of my friend.

Thus, I would like to ask you the following:

1. I work in construction and my palms (hands) have open sores very often. Every time I meet my friends we shake hands.. What if one day my hiv+ friend has a tiny cut on his palm too ? I have a family and I cannot take any chance.. In general, the virus survives a couple of minutes outside the boby. This is a "huge" interval of time for transmission to happen.

2. I visit his apartment very often to watch football or play xbox... I remember one night during the world cup we ordered pizza and I ate a piece of pizza that he left on the table. He had herpes on his lips. This means that I probably ate a small amount of his blood.

3. We play football together. We both like to play as goalkeepers and sometimes I change him during the match, i.e. we use the same gloves. There may be some blood of him inside the gloves. What if one day he has a wound or cut that he thinks it is not open ?

4. We go to the barber shop together. The barber uses a non-disposable razor to finish up some hairs on the back of our head.

There are several other situations that I can think of where transmission can be possible...

I have asked a nurse that works in an nhs clinic and she told me that hiv cannot be transmitted outside its host. The opposite opinion is a myth. But she is just a nurse. She is not an expert. I have read in an older post of yours that hiv may survive for several minutes outside its host. This means that by spending a lot of time with an hiv+ person increases your chances of acquiring aids. Am I right?

I have found some official U.S. statistics about hiv. The 3-4% of the reported cases have the status "undefined source". Maybe there are some cases of infection from a casual contact but the doctors always ask about sex. If the infected person says that he has no idea how he got it, then the doctors say that the patient is probably lying or he/she got if from an ex partner in the past.

I am very confused.. I love my friend but I do not want to catch this deadly and incurable disease.

Can you please help me on whether daily casual contacts as described above have any chance of infection? (even 0.000000000001% is a real chance for me)

I apologise to add another thread regarding needlestick injuries to this forum, however my anxiety is huge and struggling to get by day to day.

Basically i was pricked by a needle in a medical facility in a country which does not have particularly high hiv prevalence. While the area was red there was no signs of running blood. However i was unable to take PEP as it was not available and I did not know it existed. Many sources assert that the risk of catching hiv from a needlestick injury is 0.3% , i was wondering whether this figure is only relevant to those who took PEP?
Secondly, would my life be over if I did, however unlikely, catch HIV, or am I right in thinking that there are effective treatments available now which could help one live at least what appears to be a normal life.

Thank you for reading this post

What do you think about the risk of hiv transmission?
If I get tested in 28.day, and get the negative, should I close the subject?

Thanks.

Good Afternoon Doctors,

Firstly i would like to say thank you for your advice in advance.

My story goes like this; around 75 days ago i had unprotected (vaginal) sex with a CSW in the U.A.E, a mistake which i will regret for a very long time.

I decided on the 65th day that i really should go for a full STD Check including HIV, fear had kept me away until then. Thankfully all the test undertaken came back negative. I have listed out the test with the results below..

HIV 1 & 2 + P 24 : Negative
Hepatitis B: (HB "s" Ag): Negative
Hepatitis C (HCV): Negative
Syphilis (RPR): Negative
Chlamydia (Trachomatis Ag): Negative
Gonorrhea (Gram stain): Negative
Urinary Examination: Normal
Herpes (HSV): Negative

In relation to the HIV Test, my Doctor in the U.A.E has told me that the results of my HIV 1 & 2 + P24 are quite reliable at the time (65 days) and when i asked about taking a 12 week test he confirmed that this would only be for my own piece of mind and that he would be very very surprised if the result changed.  I would be extremely grateful if you could maybe provide some further clarification regarding the HIV Test and in particular its accuracy. I have tried to summarise in the questions below;

1. How accurate is the HIV 1 & 2 + P24 at 65 Days?
2. In your experience have you ever witnessed a negative HIV Test (as per above) at 65 days which has become positive at 12 weeks?
3. Is it possible for Antigen when reached its peak level 8-10 weeks, begin to vary thus become undetectable with the P24 test and at the same time Antibodies being undetectable with HIV 1 and 2 test?. I know this may sound like a daft question but this has been running through my mind on a daily basis.

Also i have not had any symptoms since exposure to this point (75 days).

Once again thank you very much for your time and i really do appreciate any advice that you are able to give.

Thank You

Hi docs

Firstly I think you do a Truely excellent job here and I would like to congratulate you

I have detailed below a post I wrote to med help and the response of Dr HHH and was wondering if you respond to my further questions as I'm still suffering with massive anxiety because of this

Post to medhelp

My exposure was 2 &1/2 weeks ago, I went to a strip club in Spain where I had a lap dance from a Romanian stripper, she danced for me whilst I was fully clothed but half way through the dance reached her hands up my shorts and through my underwear and groped my genitals for about 2 seconds, she then continued to dance for me which involved gyrating on me whilst I was fully clothed. I ejaculated in my trouses which weren't removed until I got back to the hotel, I had no visible cuts or sores however I can't be sure she didn't have vaginal fluid on her hands and for the benefit of this I would like to assume she did. i am suffering severe anxiety and panic attacks and now have dizziness and mild headaches*
If you could address my questions below it would be much appreciated*
1. Please could you offer your opinion as the the risk of my exposure*
2. Am I safe having unprotected sex with my wife*
3. Do the symptoms I have described sound like those associated with ars, if not what could they be*
4. Does the fact that I ejaculated in my trouses shortly after increase the risk*
5. If her wet fingers had made contact with my urethra what would my risk be? (I'm told the virus is inert and would not have the energy to travel into my bloodstream post exposure to air)*
6. If I were to develop more symptoms, should I take more affirmative action, I.e. Telling my wife *
7. Can HIV be transmitted via the back of the foreskin*

Dr HHH REPLIED

So by rights, your question could have been deleted without reply. *I tried entering "lap dance" into the search window and it identified 82 thread with questions pretty much like yours. *Hundreds of others asked about hand-genital contact or fingering. *The responses all were the same: *there is no risk of HIV/STD transmission, you do not need to be tested, and you can safely continue unprotected sex with your wife. *To your specific questions:*

1) This was a zero risk non-exposure event. *It doesn't matter whether or not there were vaginal secretions on her hands.*

2) If this is your only potential exposure to HIV or other STDs, there is no risk to your wife. *You cannot transmit something you don't have.*

3) Your dizziness or headache do not even hint at ARS.*

4) Of course there is no risk of ejaculating while clothed. *How could that possibly be a risk for anything?*

5) This would still be zero risk.*

6) If you develop symptoms that concern you, see a health care provider and follow his or her advice. *Any symptoms you develop cannot possibly be due to HIV or any other STD from this event.*

7) The inside of the foreskin is susceptible to HIV. *But still, hand-genital contact never transmits the virus.*

Sine this post I had a DUO ag/ab test at 28 days which was negative and am due to have the standard 6 week test with my local GUM clinic on Thursday.

My questions to you are these:

1 do you agree with HHH and what is meant by the term 'zero risk', as I understand it, this means a technical/scientific imposiblity, something I am not sure applies here?

2 I visited my GUM clinic at 2 weeks who refused me anything until the 6 week point and suggested anything before this would be pointless, why?

3 is there an official window period guide in the UK or is it left to doctor discretion, I have read you describe the DUO test as conclusive on many occasions yet all I read about is the fact ghat nothing is conclusive until the 3 month mark

4 how conclusive is my 4 week test

5 finally iv read the link to the 2008 uk guidelines on HIV testing and to my limited interpretation they seem to stipulate that the DUO test is very useful for early detection not providing conclusive negatives? What is your interpretation this, am I wrong?

I apologise if I appear cynical, it is nit my intention, I am just somebody who has perhaps spent far too much time in google and needs a few things clarifying

Thanks in advance for any help

Hi,

I had protected sex 8 weeks ago, and I had a DUO test at 41days which was negative.

My worried first is that while having protected sex, I'm an uncircumcised male, and she cut that skin of the uncircumcised male and bleed, she kept doing oral sex, my worries are realted with this, saliva or even if she was bleeding on her mouth and I didn't see.

My second worry is related with the DUO test, is very confusing online with so many people telling you different things, it loks like the test is not even approved in the US. ( im in uk )
so as I understood, p24 when not detected is because the virus has bound to this p24, this means if they disapeared the antibodies are detected, some people say that if p24 has disapeared the antibodies haven't developed enought to be detected ? so confused.

I'm worried because I had many symptoms, like body ache, I had an oral herpes after a few days of my DUO test, and now 8weeks on, apeeared 3 flat red dots on the head of my penis on the left side about 1-2mm wide, only 3 on the same side, they look like they have a litle cut in midle, I've look around and the penis looks healthy but can't explain where this dots came from, could you comment on this dots please.

Thanks in advance
