My GP diagnosed what he called hairy leukotypia on my Tongue.
He advised me to go for a HIV test. My last exposure was last May and I had two negative HIV tests at NHS GUM clinics at six and seven weeks after the incident, highly risk incident.
Do you think the forthcoming test will be positive? Is it usual this leukotypia to appear within 10 months?
Thank you for your help.
Hello,
Thank you very much for your post. I assume that you mean 'hairy leukoplakia'. This pathology is associated with Epstein-Barr virus and occurs mostly in people with HIV, both immunocompromised and immunocompetent, but it can affect patients who are HIV negative. It has been associated with HIV infection and/or immunosuppression. The risk of developing hairy leukoplakia increases with the decrease in CD4 count. It has also been described in patients with other forms of severe immunodeficiency including those associated with chemotherapy, organ transplant, and leukemia. Oral hairy leukoplakia also has been described in association with Behçet syndrome and ulcerative colitis. it has been described in patients with a normal immune system too, though it is rare.
In the context of HIV infection, it would usually happen several years after the initial infection, not ten months. Therefore I think it is highly unlikely to be the cause in your case. And you have had two negative HIV tests already.
If what you have got, it is indeed oral hairy leukoplakia, you need to have appropriate investigations to try to find out the cause.
Best wishes,
José
You asked some further questions. I note you did not specify the incident or the circumstances.

The GP doctor based on my background and the inconclusive for him HIV tests adviced for HIV testing. fair enough - although leukoplakia is easy to mistake and we haven't had a chance/opportunity to examine you so I can't comment further
Do you think there is a chance my results to have been incorrect?
same as above - I don't know what the incident was and as Jose has said, leukoplakia is usually a condition of advanced disease / time frame - and not weeks. That said I think the prospect of having two sequentially incorrectly negative HIV tests at those intervals is imporbable
Do you think this Leukoplakia to be caused by smoking?smoking bad generally - could be - if of course it IS leukoplakia in the firstplace
What tests do you suggest to undertake?an HIV DUO test - other wise see an oral surgeon and see what they say - stop smoking - smoking bad
Reading between your lines shall I not consider my HIV tests back in June conclusive and I should re-test?
As you can appreciate this comes as a surprise to me to say the least.
Can't do much I suppose. Do you think it's worth a visit to your clinic to examine the Leukoplakia? And if not Leukoplakia whatelse can it be confused with?
You do seem intent on having HIV despite the evidence to suggest that in fact you don't have it.
I wouldn't be so enthusiastic - although it is a very treatable disease it is still an awful one - so don't be silly and go wishing it on yourself.
I answered your question partly with this:-

For some perverse reason you've decided that I have said that your result is unreliable. I didn't say that - quite the reverse. I think the chances of you having two sequentially incorrectly negative HIV tests at those intervals is improbable - ie I do not believe you have HIV.
As I say, I don't know whether you have hairy leukoplakia or not - I doubt it as it can often be misdiagnosed.
You still haven't specified the risk so we seem to have made no progress there either.
For clarity, on the basis of the information you have supplied I do not believe you are HIV positive - I think you are HIV negative.
I said to you that if YOU felt the test was unreliable or false then you should test again using an HIV DUO. I do not believe you need another test.
I'm not going through the whole list of causes of hairy leukoplakia for you.
Thank you very much for coming back to me, really appreciated, indeed.
I beleive the test may be unreliable because your clinic is possibly the only one in the world wide web that categorically confirms an HIV 4th gen as absolute conclusive after a month. Almost all they advice that a fully conclusive result is after 90 days.
I don't want to have the virus since even if the virus won't kill me my anxiety, stress and fear will send me six feet under sooner than later...
I want to beleive you and I do respect your medical experience and integrity. However what these White patches at both sides of my Tongue could be?
My incident was high risk specific exposure with anal unprotected sex with a male of unknown status; can't recall if there was ejaculation.
Hope this completes the jigsaw for your on-line assessment.
In your case you had two - not one - two - 4th generation tests at 6 weeks and 7 weeks post exposure. A third generation test would give you greater than 99% accuracy at 6 weeks and you've had two tests which are better.
I remain certain that you are HIV negative.
I am closing this now as we seem to be going in circles somewhat.
