Default My Risk

Hi Doctors,

This is to follow up my previous thread, someone broke into that thread than then you locked it.

What I have written in the previous thread is:

I have had many sex actvities with escorts in the last several months, maybe 20-30 times. I understand that I have risks for STDs, but just want to know about the risk of HIV.

In all the cases, I used condoms for vaginal sex, and no breakage or slip. Sometimes I did deep kissing with the girls (sometimes I see blood after brushing my mouth, is this a concern? No blood was seen before and after the kissing though). In most of the cases, the girls gave me oral sex without condom, I never did on them.

Here are the further concerns

1. In one case, after finished, the girl used some tissue papers to take the condom off, after taking it off, some of the semen fell on my penis again from the condom. Is this a problem?

2. In another case, the girl gave me a blow job without condom first, then put a condom on for vaginal sex. I did not come, the girl then continued to gave me the blow job with the condom (the same condom), then she took off the condom and continued to gave me the blow job, then put another condom on to have vaginal sex. We also did deep kissing. This is my biggest concern.

With everything described above, should I have to worry about HIV, should I have to go for testing?

I would like to ask for some further questions. Yesterday (3 weeks after the last encounter, or 3 weeks after I stopped all the stupid activities), I started to have sore throat in the morning and then caught a cold, today I feel tired and maybe a little bit fever, but the sore throat got better (still a little bit). This has made me worry again about HIV, what is your suggestion? I understand that these days the weather is changing so quickly, and the air gets dry as well, but still worried (I live in Canada).

My another concern that I forgot to mention in the previous thread is: In one case, I visited a massage parlor, only massage and hand job were given. However, the girl ride on me, and she used her pussy to rub my balls (not the top of my penis). When she was turning over on me, her pussy might have contacted my penis, I was not sure if it has contacted the top of my penis, if that is the case, the duration should be less than one second. Should I worry about HIV in this case?

One thing I forgot to mention is that the girl took off the first condom because my penis got soft, the condom was still covering everywhere. Will this make the HIV as a risk in this case?

Sorry for making it so complicated, and I really appreciate your patience and answer. I have stopped all the stupid activities. So with everything I have described in the previous and this threads, should I worry about HIV, or should I go for a test?

I think I answered the original question.

I do not believe that you have put yourself at HIV from what you have said so far.

I do not think that you need to have an HIV test in connection with this episode.

But - an important point is that very many people with many sexual partners do need to have full regular STD and HIV tests as a routine so I would encourage you to do that.

Thanks for your reply - sorry to sound stupid, but if I am not risk for HIV, why should I test for it?

I just had an INSTI third generation test (the one gives the results immediately), and it was negative.

This is 4 weeks after the second last activity, and 2-3 weeks after the last activity, the last activity was mentioned, which is:

I just wonder, how accurate is the INSTI test? The Canadian public health website says that it is 95% accurate at 34 days and 99.6% at 3 months. Does the 95% means that there is a 5% chance to have a false negative? If that is the case, the 99.6% accuracy also does not seem very conclusive, since there will be 4 false negative in 1000 tests, am I right?

On the other hand, a nurse working at the public health told me that he has been working on this for 20 years, but never seen anyone tested at 4 weeks become positive after, he only saw people change to positive from 2 weeks. He has also never seen anyone got HIV from protected vaginal/anal and unprotected oral. What do you think about this? He told me that this HIV test for me is just for mental health, however, he still says that the unprotected oral and protected vaginal are low risk (not zero).

The nurse told me that I can come back at 3 three months, but that is just to fix the mental problem, do you agree with him?

I am sorry that your posts have not been replied to sooner. I am going to try to answer you as Dr Sean Cummings has been extremely busy!
From your description of the sexual encounter, I can assure that there was no real risk at all of HIV infection. You need to have unprotected penetration with a HIV positive person, to put yourself at risk of HIV. Therefore, in your case, I would not worry.
The INSTI test, which we user in our clinic routinely, is highly sensitive and very reliable. I would agree with the figures of reliability that you give, and is considered conclusive after 8 weeks. Many guidelines only recommend that it is fully conclusive after 12 weeks. However from my clincial experience I can say that a negative result after 4 weeks would be highly reassuring, and fully conclusive after 8 weeks. Most people who got infected tested positive already at 4 weeks. At this period of time, as we would normally use the HIV Duo test, the vast majority of these test result (in the cases of patient who had been infected), not only the p24 antigen was reactive, but also mthe antibodies (which are what one get tested for with the INSTI test).
In summary, try not to worry so much. You did not put yoruself at risk in the first place and you have had a negative INSTI test at 4 weeks. I am totally sure that you are absolutely fine and that you are HIV negative. Even though I do not think it would be necessary at all, you can repeat the INSTI test at 8 weeks (which I can assure it will be negative), you can forget about it and move on.
