Default protected mistake plz help

hi i am a 22 year old heterosexual male from london i have not been sexually active in the past so i dont have any history of risks and stds.Unfortunately this year i had two exposure with an escort in london.Now about my exposures
1st jan 2011
after new years party an encounter with a 23 year old white spanish incall escort near your clinic. she put on a condom (durex) by her mouth and gave me oral for about 5 mins then with the same condom 2-3 mins of vaginal sex and i ejaculated in it.the condom was on my penis & the semen was in its tip which i saw definitely.i didnt notice any tear but i didnt check for any leaks or invisible holes obviously. she removed it and wiped my penis with a wet tissue.
13th January 2011
same place again this time i made sure no penetrative sex.she gave me protected blowjob only and i ejaculated in it and the condom was intact throughout and on. the only contact with her vagina was with my hand i rubbed and fingered her briefly and i didnt have any visible bleeding cut or wounds.this was followed by massage on my back.
Testing:
hiv duo test 28 days after first exposure and 16 days after second exposure at a private clinic (dr lab)
hiv duo tesy 44 days after first exposure and 32 days after second exposure at PLEASE DO NOT NAME OTHER CLINICS IN YOUR POSTS - THEY TEND TO GET VERY UPSET (dr lab)
Questions :
(1) i am not concerned about my second exposure as it was almost zero risk. your opinion plz specifically on protected blowjob and hand vaginal contact?
(2) i am worried about my first exposure of protected vaginal sex as condoms are 95-98 % effective only. altough my understanding is this figure is for long term effectiveness of condoms with couples who say they use condoms regularly otherwise how on earth can someone become pregnant if the semen remains in condom what is your opinion.
(3) bashh statement in march 2010 said 12 week test should be given to everyone even on low risk while hpa nhs algorithm says 4-6 duo is conclusive. what are the real facts currently on window period guidelines in uk. i cant find clear answer anywhere are there conflicts ib hpa bashh bhiva ?
(4) sean said in one of his post in the past he had only three cases where 28 day duo test failed to pick them and they were clinically ill & had very high risk exposures. my question is when did their redult change after retest 6 8 or 12 weeks?
(5) what is the overall prevalence of hiv in female sex workers in uk dr at (do not use other clinic names) said definitely less than 1 percent what is your opinion?
(6) have u ever had a patient who got infected from female escort in uk after one exposure not trying to prove any point just want to know your experience?
(7) finally how conclusive are my results should i test again after 10-12 weeks?
(8) have you ever seen a patient turning positive after eight weeks with third or fourth generation tests?

yeah i have not developed any symptoms yet its been more than 7 weeks if it matters.

Your condom was fine - you do not need to worry.

You had a zero risk event on both occasions - you wore a condom which worked. The HPA algorithm is correct. The BASHH statement is also correct IF there was a risk, which there was not in your case. You are looking for hazards which are not there.
