receptive oral sex with ejaculation+pep. very worried
hi doc, i'm a guy from south of Italy, 6 days ago i had a sexual encounter with a married swinger couple. i practiced cunnilingus, unprotected oral sex to him with ejaculation ( swallowed ) and PROTECTED bottom anal sex.
started up pep 20 hours later ( 4 kaletra pills a day + 1 of truvada).
1) was in your opinion pep needed ?
i read that in the guide lines it's "considered" in my case.
today i took 2 kaletra pills 2 hours and half late.
2) is it very bad for the therapy ?
3) are the long and short term consequences of pep in my case not worth for the low exposure ?
i contacted the couple and they told me that their last test is old, but they're willing to make it again as soon as they come back in town and not to be worried cause they use the condom and didn't engage in "risky behaviours " since the last test .
4) if they indeed show me the results of the new test , should i interrupt the pep in your opinion ?
i considered my risk = 0.04 ( the fellatio risk ) * 0.10 ( the % he's hiv+, too low ?) = 0.004. with the pep( 0,0008 . 5) does it make sense ?

No, I do not believe that it would have been necessary as the risk is very low, especially not knowing the status of the other man

It is not advised, but can be considered. I would not have recommended it in your case

No, it would not change the effectiveness of the medicaton as you are taking the correct dosage over 24 hours.
3) are the long and short term consequences of pep in my case not worth for the low exposure ? It has no long term consequences. In the short term, it can give you side effects like nausea and diarrhoea, which can be contained with medication. Any possible liver abnormalities will recover after you finish the course, but in most cases this is not even a problem.

If the man gets a recent negative HIV test, I would advise you to stop PEP.

As you do not know thae status of this man, the estimated risk to you would be between 0% to 0.006% (between 0% to 0.04% would be if he was HIV positive, but it is much more likely that he is not). Obviosuly PEP would reduce this dramatically. Therefore I do not think that you need to be concerned.

the couple submitted 13days from the exposure to an antibodies-antygene p 24( only for hiv 1the p24) and a pcr test.
i don't know whether the PCR is RNA or DNA.
today i withdrew the ab results ( negative). for some unknown reasons the ag p24 results will come in 10 days and today i'll receive the pcr results.
how reliable is the pcr test 13 days after a possible exposure ?
is it possible that someone ( maybe elite controllers in early infection ) is negative to both antibodies and pcr 13 days after infection ?

You've asked some more questions:-
1) the PCR will be extremely reliable at this point - 13 days. If that is negative then I would stop worrying.
2) elite controllers are people who over a prolonged period of time will meet certain characteristics in terms of testing - you can forget this as an issue at this point. If the tests are negative the you forget this as a threat. You are not at risk and I would say that PEP is not indicated.
