I hope you can help me. I'm a 23 year old heterosexual male.
On July 29th 2011 (26 days ago) I foolishly had protected vaginal sex, but received unprotected oral sex (around 10 minutes) with a sex worker. She was young (20) with no visible signs of an illness, cuts in the mouth, etc.
Since this date, I have obviously felt very stressed, ashamed and paranoid about my risk of an STD, especially HIV. After doing much reading on the internet, I have mixed feelings and would love some clarification/piece of mind and risk analysis as to my situation. I broke down my concerns into numbered points if you don't mind.
1. About 3 days after the 'contact', I got a pain in my throat area, not a sore throat when swallowing but a pain that sometimes occurred (felt more muscular) that did swap from right to left. It went away pretty quickly.
2. Around day 9, I developed a 'skin irritation' on the lateral parts of both of my thighs (localised to only this area, and primarily on the lift side). The majority of the bumps were very small and only skin coloured. The area is not red or inflammned, nor is it itchy, nor painful or pussy. There were a few red ones but nothing dark in colour like is mentioned with regards to an HIV or macuopapular rash. I have read on sites that folliculitis and an increase in skin issues (increased pimple development which i've had on my back) can arise during acute seroconversion. This irritation went away about a week ago.
3. Around day 14, my paranoia really kicked in about swollen lymph glands, especially around the neck. I felt the area behind my jaw ache when i first opened my mouth to chew (this has happened before) and started getting aches around my TMJ. I know the parotid gland lies here and it seems to be a little swollen, as does my submandibular glands. I know this is also a possible sign of swollen lymph glands and HIV. Is this a genuine concern regarding my risk of exposure to HIV?
4. Are an increase in pimples a valid worry with regards to HIV or an STD? They are on my back, congregated in different areas (mainly left shoulder blade - 12-15 present).
5. I had a bad acute inflammatory reaction in my hip flexors at a training session last week (Tuesday) and after having that happen, I noticed that i could palpate and feel my groin lymph nodes (or that is what i think they are). They are small (from size of a pin head to the size of a pea) and relatively firm and mobile. I know groin lymph node swelling is a sign of HIV and I am very worried about this. I have been able to palpate and feel these in the past (I am quite thin - 6ft 1, 74kg) but would just like to know your opinion.
I have not had any high fever, no headaches (besides it being so hot and humid right now from dehydration), muscle pains (besides those from physical training), and no constant diaorrhea. Also no pain experienced when urinating, no blood in urine, no testicular pain and have had some abdominal cramps but nothing significant.
In august 2010 I did catch chlamydia from oral sex with a sex worker (also had protected vaginal sex and unprotected oral with her). Had HIV tests done for that and was NEGATIVE as of july 22 of this year.
I went to see a sexual health doctor on day 14, but he was useless and offered me no insight or reassurance at all regarding my risk level and situation regarding my symptoms.
I do apologise for the mass of information, but I just wanted to tell you everything so that it would help you.
My sincerest appreciation for your time and help with my situation.
Relax. You will not have HIV from this episode - even if the lady had it which is unlikely.
The symptoms you describe are irrelevant and not related to HIV in respect of the unprotected oral episode.
You are right to draw attention to the fact that you can acquire other STD's etc such as chlamydia, gonorrhoea etc from being sucked if the other person has it in their mouths, but you will not acquire HIV from being sucked.
The answer to your worries is 1) to remain calm and 2) to have an HIV DUO test at the 28 day mark. Many if not all NHS units will offer this test so you do not necessarily need to pay for it. I am sure it will be negative.
Unfortunately I live overseas in Hong Kong and so do not have access to the NHS units with regards to an HIV DUO test. What does this test actually 'test'?
I know from past STD panels that I can have HIV, hep, syphilis, gonorrhoea, chlamydia and herpes assessed. Are any of these of great significance regarding my symptomology?
You are right - I am suggesting the HIV DUO not because I think you have HIV but because you are worrying about it - so it is for reassurance.
I do believe you should be tested for the other STD's as per my original reply. I do not believe the other STD's are manifesting in the way you describe though.
