Default Very worried - please help

Hello Doctor,

Yesterday - I had a night with two sex workers. I am male, they both were female.

No penetrative sex occured.

One of the workers licked ans sucked my testicles initially - obviously no protection there.

She went on the massage/mastrubate my penis which had no condom on at the time to obtain an erection.

This went on for a while - mastrubating my penis without any protection.

There was a lot of caressing and touching of other parts of bodies with each others hands. I did not touch their vaginas.

The two sex workers had a dildo which I held and used on them.

I then wore a condom and oral sex was performed on me. There was no ejaculation - and my erection remained with the condom on. All the while caressing and touching the two while they did the same to me - but I did not touch their vaginas and my fingers did NOT came in contact with any vaginal fluids.

I eventually ejaculated into the condom and it appeared to be fine since it held all fluids. I removed the condom and disposed of it.

My concern is, I was fondling and caressing the two bodies (not the vaginas) and had oral protected sex performed on me - the condom was kept on for longer until ejaculation - which I then removed. As explained above - to obtain an erection, one of them licked and sucked my testicles before protection was put on and mastrubated me.

The underside of my penis was a bit sore from the previous night due to friction of excessive mastrubation.

Today I am feeling very worried with what happened yesterday.

Am I at risk of HSV-2, HIV, Syphillis, etc. or any other STD from this episode?

Please help.

No - there is no risk here. You didn't do anything remotely risky.

Thank you for your reply Doctor.

Apologies for posting my question twice - I was quite anxious.

One follow up question I have is regarding exposure to Syphillis (and HSV-2) - I realise you say there is no risk in my scenario - but my understanding of Syphillis (and HSV-2 to a different extent) was that it transmits due to body to body contact - and in that respect - since we did caress each others bodies - (although as explained above - I did not come in contact with their vaginas) - would that not be a risk in my case?

Similarly - kissing and sucking breasts - would that not expose one to Syphillis or HSV-2?

Please excuse my questions if they appear to be silly - I just need to understand things for my peace of mind.

Hello,
It is highly unlikely as you would need to have genital to genital or mouth to genital contact without protection, in the case that they had either syphilis or genital herpes (HSV-2), and that does not seem to be the case in this sexual encounter.

Thank you very much for your reply. I am probably being paranoid about this - but just want to ask if it would do any harm if I took a prophylactic 10 day course of Tetracyclin - 100mg every 12 hours. Would this cause any harm? I think this would cover almost all the common STI bacterial infection possibilities except Syphillis. Am I right in thinking this?

I do not beleive this to be necessary. However tetracyclin would only cover some STIs. It would not cover Gonorrhoea or Syphilis. And Chlamydia is better treated with either azithromycin or doxycycline.
A good combination would be a single dose of azithromycin 1g, together with a single injection of ceftriaxone 500mg.
