Question Weird Case in Thailand???

Hi Doc,

Two weeks ago, I was in Thailand. I got pretty drunk and had sex with a sex worker. During the session, a few things happened and it made me very nervous of contacting HIV:
1. We had unprotected oral sex both ways (she on me and me on her) and protected anal and vaginal sex.
2. During the anal and vaginal sex (I couldn't remember exactly which one), the condom did slipped out. However, we did immediately put it back onto my penis and continued the session.
3. At the end of the session, she masturbated me till I ejaculate. However, during that period, she used a condom to finger my anus. What I am concern here is that, because I was drunk, I couldn't remember if she used a new condom or used back the same condom, which I had just used shortly earlier to penetrate her anus and vaginal.
4. When I wake up in the morning, when I was a little more sober, I saw certain signs that made me suspect that she might be a transgender (man to woman after the full operation), although she kept denying.

I know the whole fiesta sound stupid, or even funny. However, the whole thing is bothering me a lot and I am having sleepless nights worrying about HIV. My questions are:
1. Looking at the whole episode, what is the chance for me getting infected with HIV, if she is a HIV/AIDS carrier?
2. If she had finger my anus using back the same condom, which I had just used shortly earlier to penetrate her anus and vaginal, will my chance of getting infected higher than normal, if she is a HIV/AIDS carrier?
3. Any idea, what is the chance (in terms of %) that she (a potential transgender sex worker in Thailand) is a HIV/AIDS carrier?
4. My son loves to playfully bite me on the cheek. This morning, before I can stop him, he bit me accidentally on a very mild bleeding pimple on my cheek. If I am infected with HIV, what is the chance of infecting him?
5. If I go for a OraQuick or Quick Blood Test, one month after the exposure, what is the accuracy of the test? If tested negatve, what is the chance of me being HIV free? Should I go for a 2nd test again 3 months after exposure?

Thank you very much for your post. I will try to answer each of your questions in 'red' here below:

I think the risk is extremely low. However if there was some unprotected penetration, there is always a risk, but this exposure seems to have been very brief. All in all, assuming that she was positive, the risk for you if you had had unrpotected vaginal intercourse would have been less than 0.1% or less than 1 in 1,000.

2. If she had finger my anus using back the same condom, which I had just used shortly earlier to penetrate her anus and vaginal, will my chance of getting infected higher than normal, if she is a HIV/AIDS carrier?

There is a possible risk, if she was rough with her finger, but I still believe that the risk is very low for you.

3. Any idea, what is the chance (in terms of %) that she (a potential transgender sex worker in Thailand) is a HIV/AIDS carrier?

Incidence of HIV among sex workers in Thailand is very high, especially high among male and transgender sex workers. Around 1.5 to 2% of the adult population in Thailand are HIV positive (over 700,000 people are HIV positive), which is a very serious problem, even though Thailand is one of the few developing countries that is doing the utmost to reduce new diagnoses and things seem to be improving.

No need to worry about this; no risk for your child.

5. If I go for a OraQuick or Quick Blood Test, one month after the exposure, what is the accuracy of the test? If tested negatve, what is the chance of me being HIV free? Should I go for a 2nd test again 3 months after exposure?

Even though a negative test result at one month would be highly reassuring, it cannot be considered conclusive. You would need to wait at least 8 weeks. However you could always have a HIV Duo at 4 weeks and this will be totally conclusive. I would recommend you having this so that you can get peace of mind as soon as possible.

At least I felt slightly better now. However, there is still this worry in me, probably due to guilt. Hence, please bare with me for just a few questions more.

Base on my description of the incident, with consideration of the following:
A) There should be NO unprotected penetration or vaginal/anal intercourse as I remembered. The condom was still covering part of the top of my penis when it slipped and we wore it back immediately.
B) We had perform unprotected oral sex on each other, but me on her in less than 2 minutes.
C) She may (or hopefully may not) have finger my anus using back the same condom, which I had just used shortly earlier to penetrate her anus and vaginal,
D) She is a sex worker (potential transgender that had went through the full operation) in Thailand.

Questions:
1. What is my overall chance (in terms of %) of getting infected with HIV, not knowing if she is HIV positive?
2. What is my overall chance (in terms of %) of getting infected with HIV, if she is HIV positive?
3. My wife wanted to have sex with me recently and I dare not. Is it ok to have sex with my wife? If not, what about just oral sex (typically unprotected though)?

Let me try to answer your questions here below, even though it is difficult to give you an accurate answer and it will be an estimated risk only, as there are so many different variables:
1. What is my overall chance (in terms of %) of getting infected with HIV, not knowing if she is HIV positive?

This would be assuming that there was some unprotected penetration, the risk for you would be less than 0.005% (less than 1 in 20,000)-this would be in London; risk might be slightly higher due to the higher incidence of HIV in Thailand, but it would still be very low.

2. What is my overall chance (in terms of %) of getting infected with HIV, if she is HIV positive?

This would be approximately less than 0.1% or 1 in 1,000

3. My wife wanted to have sex with me recently and I dare not. Is it ok to have sex with my wife? If not, what about just oral sex (typically unprotected though)?

I think you should be tested first, even if there is such a small risk. More importantly you should be tested for the most common STDs, such Chlamydia and Gonorrhoea, which are very easy to trasmit and can be contracted through oral sex

One more clarification needed so as to have a closure on this...

As mentioned in point A) above, there was NO unprotected penetration nor unprotected vaginal/anal intercourse as far as I remembered, because the condom was still covering part of the top of my penis when it slipped and we wore it back immediately. Also, I don't remember having any unprotected penetrative sex with her during the whole session.

Questions:
1. Did any point in my description makes you think that I might have exposed myself to unprotected penetration without myself knowing? If yes, please help elaborate.
2. Also, if there was NO unprotected penetration, what's the overall risk of HIV, considering all the other things (points B, C & D) that had happened during the session?
3. To conclude, I will be going for test in the 6th weeks. However, just to understand my risk for now, I would suppose my risk to HIV is very very low. Is that correct?

I think my answers have covered all these points already.
If you yourself are not sure about what exactly happened, I cannot really comment on that as I was not there...You must know if there was unprotected penetration or not. If you are not sure, that is obviosuly a different matter and a different risk all together.
If you are totally certain that there was no unprotected penetration, I would certainly estimate the risk to be extremely low or even none at all.
