Default worried after the tests

Dear Doctor

I am a 36 year old married male and generally on good health. I had taken a course of Tami flu (8 day course) in 1st week of September, 2010 as a case of prevention. I never use any kind of drugs and not in the men.

On February, 20 this year I have visited a Chinese massage parlor in Dubai and had protected both oral and vaginal sex with the girl after a short massage. After I ejaculated in the condom I noticed some vaginal secretions on my pubic area. I got panic as I had an one day old cut under my penis – on the skin of the balls just below the penis (it was not bleeding but has a white substance on it) and I didn’t check if there was any secretions as I was freaked out. I immediately washed the area with soap and water. The girl assured me she is clean and nothing to worry and I also called the owner of the parlor and she also assured me nothing to worry and the girls are being checked every month!

I started to check on the internet for the symptoms of HIV as I was worried a lot and started to find out the swollen lymph nodes on the neck as an early sign with poking on the neck around 20-30 times a day. On day 7 I found a swollen lymph node on my neck, it was like a tube from the front of the neck to the back side not like a bean!. After few days more I start to feel itching and burning sensation all over the body and there were no rash or fever (I never had fever till today) which eventually stopped after 2-3 weeks. On day 16 I had a mild sore throat which went away in one day with drinking hot water. I had some small pain full blisters randomly on my body from week 4-6. The swollen lymph node in the neck has vanished almost but still I can feel a bit of it if I try to find it (I am on week 8.5 now). I have been to an STD specialist from week 3 till today.

I had

HIV DUO after finishing day 24 – Negative

HEP B (Hbsag), HEP C (HCV –anti body), Syphilis on day 28 - all negative

Herpes (HSV –II) on day 28 - positive

HIV DUO after finishing day 38 – Negative

HEP B (Hbsag) on day 56 – Negative (started the first dose of vaccine)

My doctor says the small pain full blisters are from herpes, which as per him is from some previous encounters and may have my wife already have it and he said he couldn’t find any swollen lymph nodes in my body and what I am feeling is just from poking and stop poking on it. He says the itching sensation and burning sensation I had was due to anxiety and he prescribed antidepressant for me and also a medicine for the allergy. He says I am in clear from every thing and nothing to worry and no need to retest. I am not fully worry free from the conflicting information from the internet. Please try to help me out to evaluate my risk and the accuracy of my all tests considering the Tami flu course.

Hello

The tests have established that you do not have HIV.

They have also have shown that you do have Herpes II

Your doctor is correct.

The Tamiflu is irrelevant and has not and will not affect the HIV tests etc.

You do not have HIV.

Default please help

Thank you very much for the assessment and reassurance doctor sean.

I was expecting little more explanations on the other tests done (HEP B, HEP C, SYPHILIS) with the time frame and in connection with the HIV test done. HEP C anti body resut on dat 28 was 0.256 where cut off was 1.00. Also I am expecting the reasons for the so called swollen tubular thing on my neck and the ithcing and burning sensation i had. I still feel itchy on the body here and there some times. Do you think i need to do any more tests in connection with this episode. Don't take it wrong I am very anxious person and was thinking of suicide till i get my first negative results. And I didn't have more than 3 hours sleep at night after this incident till today. That's why I come up with this reply. Hope you can understand.

Thank you for the reply,

I was expecting some more explanations for the doubts I had. What about the reliability of the syphilis and hepatitis c test done at day 28? and the the reason for the enlarged stuff on the neck and the itching of body. Please tell me if these tests sound ok or should I do a retest again. Actually I posted a follow up question on yesterday and i didn't get any reply.

Good Morning

You had protected sex and have had a negative sequence of tests. The combination of these two factors means that you do not have the diseases you have asked about in your original question. Note that your further questions are different so whilst you may have been expecting a different reply you didn't ask the question in the first place.

Your tests were:--

HEP B (Hbsag), HEP C (HCV –anti body), Syphilis on day 28 - all negative again, these tests are fine - the syphilis and hep B particularly are good - the Hep C is a little early and to be absolutely strict about it then a third generation Hep C antibody test is best done at 3 months. That said, hep c transmission via sexual activity is very rare and again you had protected sex = no risk of hep c

Herpes (HSV –II) on day 28 - positive test will be correct

HIV DUO after finishing day 38 – Negativecertain = you are definitely HIV negative and you do not need to repeat the hiv tests

HEP B (Hbsag) on day 56 – Negative (started the first dose of vaccine) again, perfect - and well done for starting the vaccination sequence.
I don't have an opinion to offer on the "tubular" thing. You've been seen by your doctor who seems to have cared for you in a very professional and appropriate way. I haven't seen or examined you and I won't speculate on this basis.
