Hi Dr Sean,

I am continuing my conversation a follow-up from my last dicussion - Condom ripped with sex worker - testing


It has been 33 days since my exposure and I have undertaken a HIV test (HIV 1 and 2 ab/ag) on the 28th day and have come back negative ... yet yesterday I realized that my lymph nodes in my neck were swollen and my ears are blocked .... I have no other symptoms at the moment apart from this .. not even a typical sinus infection where I would have a blocked nose or sore throat ..

Whats your opinion on this situation and also have you ever had someone test negative at 28 days and then come back later as a positive ?

My close friend has a upper respiratory tract infection who I have been with everyday for a while, yet they do not show signs of enlarged lymph nodes and they have a bad cough etc which I do not have.

Is it common to not show the general symptoms such as a high fever, night sweats or rash yet show enlarged lymph nodes and the feeling of blocked ears ??


Also if I got any symptoms a couple of days after the result, would it have been more likely to have shown up in the test at 28 days due to antigens etc ?

Also finally, when I was getting my test done the woman taking my blood said that they are only testing for antibodies .. yet my test says HIV 1 and 2 ab/ag and the doctor said it was a combo test .... which source is more reliable ?



What is your opinion of my slightly enlarged lymph nodes and blocked ears? what are my chances to testing positive after a 28 day HIV 1 and 2 ab/ag test ??

No, we have not had a situation where someone has tested truly at 28 days post event and initially been negative then turn into an HIV positive

What is your opinion of my slightly enlarged lymph nodes and blocked ears? what are my chances to testing positive after a 28 day HIV 1 and 2 ab/ag test ??your ears etc are symptoms of a common cold as mentioned above - the chances of a correct result using an HIV combined antibody/antigen test are greater than 99.89%
It seems clear that you are HIV negative - if you think the test is incorrect then the obvious simple thing to do is to test again and confirm!

Hi Dr Sean.

I have started even panicking more as now I have developed a small raised type rash through my chest and have seen a doctor and he seemed unsure what it was (he did not specialize in HIV and didn't seem knowledgeable in many things .....)

My rash is similar to this (SORRY THIS LINK HAS BEEN DISABLED) yet much more mild and mainly through my chest area .. I am fairly hairy and was wondering if this can be relate to any hair problems ??

Also another thing I am wondering about is that I have not had any symptoms of a cold other than a dry odd cough which is supposedly related to HIV .. and I have had some deep intense headaches through my temple (I have never really gotten headaches in that area, they usually occur in the back of the head for me)

All up just after my 28 day test an result given, I have gotten swollen lymph nodes in my neck ... the feeling of pressure in my ears and sometimes in my cheeks .. deep intense headache in my temple and head .. my head generally feels like its containing alot of pressure ... and an odd rash that I cant be 100 percent sure that was not there before but is fairly mild ... does this sound like HIV symptoms to you ? Also in regards with the swollen lymph nodes that is related to HIV and rashes that occur how severe are they to be related to HIV ? because it seems like they have swollen up with no real side effect of a cold such runny noise etc other than a dry odd cough which is also a symptom of acute hiv.

I went to the sexual health clinic and they had told me that the rashes do not seem to be HIV related and that if they were they would generally be a lot more severe and through the body .. and that I look too healthy to be showing symptoms of HIV ...other than feeling generally tired overall .. I don't sick apart from the symptoms I highlighted ... is what she said true in your opinion .. Should I be feeling alot more sick than feeling fine other than small symptoms if I were getting the symptoms from hiv ??

Also I am undertaking another test at 37 days .. if this is negative should I consider this generally conclusive ?.. and could this test be positive now as I have some symptoms 28 days after ? how long would it take to be positive after symptoms ?.... do people need to show symptoms before their test becomes positive ?? I feel like the test I had on the 28th day was not positive as I did not show any symptoms yet now that I may be showing symptoms it could turn positive .. could this be the case??

What is your general opinion on everything and the other symptoms??

I've answered all this already and I don't have anything to add. You've had a negative DUO test = you are HIV negative. This new stuff doesn't concern me.

Okay.. Also last question -

Is there any differences in the combo tests ? are they all 28 days at 99% ? I have seen a post here from you saying you were not sure about some type of other DUO test (4th Generation Hiv Duo rapid test) and its reliability .. are all the duo tests at basically 99% now at 28 days ? becuase I am not sure what type of exact duo test mine was .. it just says HIV 1 and 2 and Ab/Ag .. and I am just freaking out because I am showing alot of symptoms even mouth ulcers etc so I am pretty worried.

I think they are all likely to be of the same sort of accuracy. I am though only deeply familiar with the COBAS or the VIDAS tests. The others you will have to investigate yourself.

Obviously, if you remain concerned that you may be displaying HIV symptoms then have a further HIV test.
