Default Very Worried - Need Advice

Dear Doctors,

Please can you help me.

I met with an escort on 18/02 noon, do not know her health status. There was NO penetrative sex involved, no kissing on lips, no giving her oral. She only gave me oral (protected).

However, am worried sick as a few things happened that I am scared of now. My questions are

(1) She was fingering herself and when she was ready to give me oral, stroked my penis which had some precum. Is there any health risk here as her fluids might have touched my penis and mixed with my precum?

(2) I had touched her between her legs (just stroking her vagina) but I did not insert my finger. I had a paper cut from Friday night on one of my fingers and I am not sure if I felt her with that finger. Are there any health risks if I had felt her with my cut finger?

(3) After the oral, I did not check the condom for a cut or hole after the encounter. If by chance, there was a hole in the condom, does that pose a risk?

(4) Should I start PEP as the episode is under 48 hrs?

(5) I will have HIV Duo Test done after 28days. However, I am very scared and would like to be put out of my misery. Should I have the HIV RNA PCR test done sooner?

(6) Do you suggest that I get tested for any other STD?

Thank you for your post. I will try to answer each of your questions here below:

Is there any health risk here as her fluids might have touched my penis and mixed with my precum? I think it is highly unlikely that you have contracted anything that way. If anyhting, you might have contracted the common and easily transmissible infections like Chlamydia and Gonorrhoea

(3) After the oral, I did not check the condom for a cut or hole after the encounter. If by chance, there was a hole in the condom, does that pose a risk? No riks of HIV. Possible risk of Chlamydi and Gonorrhoea, even though highly unlikely

(4) Should I start PEP as the episode is under 48 hrs? Certainly not. No risk of HIV at all whatsoever.

(5) I will have HIV Duo Test done after 28days. However, I am very scared and would like to be put out of my misery. Should I have the HIV RNA PCR test done sooner? I do not believe that you need any HIV test. Having a test is up to you and the only justification is for your peace of mind, and not because you need it.

(6) Do you suggest that I get tested for any other STD?
You probably do not need any tests. If anything I would only recommend you checking for Chlamydia and Gonorrhoea, because they are common and are very easy to transmit

Thank you for your response, I will get the test done for Chlamydia and Gonorrhoea (along with HIV for peace of mind).

The last few days have been very stressful. I have a few further questions please that I need some help from you.

(1) I have noticed a few small dark purple spots on my penis head. Is this a symptom of some infection?
(2) Have been feeling itchy on my chest, stomach, knees, shoulders and back of hands (along with a few general body areas). Would this be a symptom of some infection?
(3) I am a bit worried about contacting Herpes and Syphillis. Is it just normal skin contact or contact with sores? I did not see any soar on her body and assuming she had some sores which I missed, can the infection be transferred through touch of sore to any part of my body? Or does it have to be contact between sore and my genitals?

Is there a rash. Syphilis can sometimes cause a rash; however it is not usually itchy. My feeling is that this has absolutely nothing to do with an STD. It might be a skin problem, an allergy, etc.

They are usually transmitted when genitals or mouth come into contact with a herpetic lesion or a syphilitic chancre. Herpes can be contracted in the areas around the mouth and around the genitals as well. I think in your case, this seems quite unlikely
