6 months ago I had mutual oral sex with a guy. Had a real bout of anxiety over what I did (and still do). I had all of the tests and got very panicky re the HIV test at 4 weeks, which was negative. I wrote to you before and you told me the result was conclusive so when the 12 week one came back negative I was ecstatic.
The problem now is.. about 2 weeks after the event I started to get achey testicles mainly the right one, this is still there now 6 months later. I have seen a urologist who’s done all the tests which are negative he thought that I had epididymitis and gave me 2 weeks ciproflaxin which made no difference.
My real question is that even though I tested negative for Chlamydia and Gonnorea could the result be wrong? I had the test at 6 days post exposure and then about 3 weeks later.
I also remember that even though it was over an hour since I peed when I did the sample, I think I may have peed first in the loo before filling the pot. Only the first burst though.
Having consulted Dr Google some websites say chlamydia can't be passed on via oral and some say they can - some say it is possible but rare. ????
I am not having any burning, discharge or anything else, just aching testicles.
Do you think that my mid stream sample would have caused the negative? Should I get tested again.

Thank you very much for your post and welcome to our forum.
We see many patients that do contract Chlamydia and/or Gonorrhoea in their throats (mainly women and gay men) after performing a blow job. If they subsequently perform oral sex on someone else (and not many people use a condom when performing a blow job), it is highly likely that they are going to pass this onto the recipient's urethra. However these infections are very easy to detect with a PCR test of a urine sample, which needs to be in the bladder fro at least a couple of hours and needs to be 'first catch' or 'first stream' for the test to be fully reliable. It is already detectable after a few days. Therefore I can only assume that this would at least have been picked up by one of the two tests that you had. Epidydimitis can be caused by Chlamydia and Gonorrhoea, but also by many other micro-organisms such as Mycoplasma, Ureaplasma, trichomona, Gardnerella, coliforms (bacteria from the gut), syphilis (though rare), Pseudomona, Mumps, other viruses and bacteria, etc...There are also non-infective causes. If your symptoms persist, you might want to test for some of these, but most improtantly, you need to have an ultrasound of your testicles. You should probably go back to your urologist for a review.

You mentioned that I should have an ultrasound - this was done and there was nothing found.
With regarding to the test that I had at 6 days I did a urine test plus they took a swab from inside the urethra. These came back negative. would that be conclusive even if it wasn't first burst?
Would the two urine tests only show positive from first burst?
Had a throat swab too at 6 days post exposure all clear.
I have had several routine urine tests and these have not shown anything, would they have shown up any of the other things you mentioned?
The urologist told me that as all tests were negative ( had the camera inside the bladder which also looked at the prostate allOk) that it was a case of either live with the pain or have it chopped off!!!
Could stress cause such a pain because I must admit it has been a bit better since I started to get some positive comments from professionals on line.
I don't know whether to go back to the GUM for retests or not what would you recommend?

It sounds to me that there isn't anything seriously wrong with your testicles and stress and anxiety can certainly cause some of these symptoms. If you are in doubt about the suitability of the tests that you had, it might be a good idea to have them repeated, even though I think that they are most likely to be negative.
