Unhappy Post PEP

Hi Dr. Sean

About two month ago I had posted a thread about my concerns. I had an issue where i had put the condom on reverse on a casual partner then i had put it on the correct way then we had sex, where i was the receptive partner. Well, as discussed I was taking Combivir after 36 hours from the episode (Commenced PEP 1/09/09 finished 03/10/09). I recall you had advised me that a 3 drug combo was required but reminded me that you wouldn't indicate taking PEP.I tried to convince my Dr to prescribe me Kaletra aswell but she said the two drug combo (Combivir) were as effective and its based on a risk assessment I had finished my course on Saturday (3/10/09). While I was on PEP on the final week I also noticed that I had tinea cruris.

I had my gp assessment and he confirmed that it was tinea. Im currently using Miconazole cream. I also had a mild fever and sore throat after completing PEP, which the sore throat is still persistent. I occasionally get a headache and myalgia. I’ve been monitoring my temperature and seems borderline. I’m also noticing that I frequently get mouth ulcers. I also have nasal congestion which is chronic and I use nasal corticosteroids and decongestants as required. I recently commenced Abbocillin VK 500mg twice daily for the sore throat. I seem to have a tender neck. I haven’t noticed any rashes on my trunk but I get dark spots that disappear and return frequently. Also for the past two weeks I had an upset stomach and now it seems better. I had frequent boats of going to the toilet although my stools are normal now.


I had a 6 week HIV blood (18/10/09) and syphilis test using the ELISA. I’m glad the results came out negative. Although, I also need to get a blood serology at 12 weeks and 6 months. I’m still very scared, although I try to maintain a SAFE sexual act at all times.

While I was on PEP I still continued to have sexual activity. I’ve asked all guys there HIV test prior to any sexual act, they seem to claim they are HIV negative, but im not sure of their status. They are all casual partners.

11/09/09 – Brief Oral sex – receptive and insertive no ejaculation, frottage,kissing
18/09/09 – Insertive oral sex, frottage,kissing
19/10/09 – protected receptive anal intercourse and Insertive oral sex,kissing
26/10/09 – Protected receptive Anal intercourse and kissing

I’m really worried I hope you could give me an idea on what’s happening.

I hope im not seroconverting. Do my symptoms resemble seroconversion?

Is it good sign to obtain negative results at 6 weeks?

Hi Nathan,

I don't think you are HIV positive and as you say, I was not convinced you needed PEP in the first place.

I think these things you are reporting are incidental and irrelevant.

I think the tests you've had already will ultimately prove to be correct - but obviously the Guidelines suggest testing at 3 months post PEP.

What I can say is that we do perform very many hundreds of HIV tests post completion of PEP early for reassurance purposes. When these are negative we have not then had any that hae gone on to become positive from the same episode.

I think you'll be fine.

Hi Dr Sean,

Since finishing up on PEP, my sore throat has been recurrent and i have been experiencing persistent coughing. Other symptoms which have been troubling me include persistent upper limb pain/numbness and sometimes tingling. It was initially one arm now its both. The lymph nodes on my neck and elbows are swollen and feel tender. The coughing is becoming very troubling, i was put on a short burst of oral steroids to settle it down. It hasn't done much. I've also had oral swabs for possible strep throat but they have come NAD. Again the antibiotics did not make any difference. I feel the nature is viral. The viral like URT infection symptoms are generally consistent on and off. I also have been getting persistent mouth ulcers and headaches. I've also been feeling unwell in general for a long time. I'm also suffering from anxiety and that has been controlled with counseling. I also recently had a chest x-ray and seems fine.

I'm currently on:

Nasonex (mometasone furoate)
Otrivin (Xylometazoline)
Turbuhaler(budesonide)


On the 30/11/09 I'll be having my 3 month HIV follow up. I know my exposure was of low risk although i'm really petrified.

1. Are these symptoms consistent with HIV seroconversion?

2. What is a possible cause for this conglomeration of symptoms?

3. Who should i be seen by? ENT or respiratory specialist?

4. I recently noticed bump or raised elevation on my head? Is this relevant with regard to HIV?

I'm very worried and i'm really finding it hard to maintain sanity. I believe everyone here is the same.
I'd be very happy if i could hear from you soon.

Nasal symptoms almost consistently rule out an HIV seroconversion illness.

Nathan, you're not HIV positive and all these symptoms are either minor and irrelevant or a mix of anxiety and a cold. You dont need to see an ENT specialist or anyone else. Have an HIV test and get your mind straight. All these symptoms will then go away.

Hi Dr. Sean,

I had my HIV test on the 27th instead of the 30th as discussed in my previous post. Last Thursday I developed rashes on my chest. They are localized around the cleavage lines between my chest and abdomen and there are a few on my left chest. I showed him my rashes and he said it might be fungal or due to hot and humid weather. Over the entire year i have gained 20 kgs, I am 5ft 7 and weigh 85kg. I use to be 62kg. Since, I have been experiencing chronic coughing with rhinitis, I had seen a specialist concerning these ailments. He suggested that i might have GORD as well. He indicated that I should take Proton Pump Inhibitors. Yesterday I had called the clinic to obtain my results, the nurse only gave partially my results and indicated that the syphilis was negative. Although, when i had asked for the HIV results she had said she couldn't disclose that information as it was against the law. Today, I chatted with my G.P over the phone, and he said Nathan its standard procedure and we shouldn't be giving out results over the phone. Since, I have been working i haven't had the opportunity to receive to pick up my results, although i will be going tomorrow. In the past the same Dr, had given my HIV results over the phone, I'm really petrified. I don't know why he just didn't disclose the information. I believe I have HIV, otherwise why wouldn't he disclose it over the phone? Its probably to prevent a negative feedback and doesn't wish to be liable. Everything I have discussed so far with yourself is accurate. I haven't performed any other sexual act in the interim. I will make one last post regarding the outcome I really hope its negative.

What are your thoughts?

Can having GORD influence oral sex even when there is no ejaculation?

I'm really sorry for my continuous message threads. I really appreciate your view.

Hi Nathan

Obviously I can't comment on your doctor /nurse etc and I don't know why they don't give the results on the phone. We give them by phone/email/text/ in person etc so no real idea.

If you are HIV positive you'll be the first seroconverter to put on 20 kg I've ever met.

Let us know the result - bet you're negative.

Mate,

Can I just ask you about your elbows! Mine are also up, on the little finger side. Little bumps. Have you spent hours searching the net- leaning on them. My doc seems think thats caused my bumps- not burstitis, more of an RSI through leaning typing, general over use of the old digits (fingers) Im worried that the bumps are lymph nodes. But everyone I speak to disagrees. Do you think maybe, just maybe, we have inadvertantly cause our elbows too swell? I get mouth ulcers too, and I have a lymph up behind my ear.

Dude. I tested negative on the HIV DUO at:

6 day , 21 days, 6 weeks, 10 weeks and then an HIV insti at 13 weeks. Do you think I have it? No! Prob not mate. I guess our "symptoms" are all anxiety related. Lymphs up through lowered immunity through anxiety- oppurtunistic bacterial infections maybe? Its nice too see another worried well with the same symptoms as myself. If you think I cant have it- then I dont think you can either mate!

It does sound like you are building this into something bigger than it is, if as Sean has said and it is unlikely that you have HIV from your activity. It would be wise to listen, try not to worry, worrying won't make the situation any better, and could well just give you anxiety symptoms, causing you to worry more..thinking that things are developing.

If you results are negative, thats fantastic, but if they were positive, what use would worrying do, - will it get them any quicker NO, will it stop the developement NO, once you have your results, ( if you are going to get them in person, perhaps take a good friend someone you can confide in, someone who can take in all the information, because you will be tense), then go out and celebrate afterwards, Whatever the result, you are still alive and still kicking, so just chill.

Hi Dr. Sean,

I received my results yesterday and I'm very happy that they are 'negative'. Thanks again for your support, wisdom and patience.

Since, I have GORD does it increase my likelihood of HIV transmission If i perform oral sex with an unknown status person? Is PEP justified?
